RELEASE OF LIABILITY AND WAIVER INFORMATION
PLEASE READ CAREFULLY. THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS.
The enrolled participant(s) and/or the parent/guardian of the participant(s) agree and understand that ice hockey is a HAZARDOUS activity. The participant recognizes that there are risks inherent in Ice Hockey, including but not limited to paralyzing injuries and death.
The participant hereby agrees to participate in the FLAG Hockey summer skating program and hereby agrees to indemnify and hold harmless the FLAG Hockey, its instructors, owners, management, agents, and employees against any liability resulting from any injury that may occur to the participant while participating in the hockey training program. The participant also agrees to indemnify FLAG Hockey for any damages incurred arising from any claims, demand, action, or cause of action by the participant.
The participant authorizes any representative of the FLAG Hockey to have the participant treated in any medical emergency during their participation in the hockey training program. Further, the participant and/or parent/guardian agree to pay all costs associated with medical care and transportation for the participant.
FLAG Hockey (FLAG) has a responsibility to its employees, participants, and the community to provide a safe, supportive, respectful, and enjoyable environment.
Because we value staff, volunteers, and participants, it is the policy of FLAG that all individuals are individually and collectively responsible for ensuring that FLAG is free from harassment or bullying of any kind. Harassment is defined as verbal, visual, written, social media, or physical conduct which has the intent or effect of interfering with an individual’s or group’s participation in FLAG activities or which creates an intimidating, hostile, or offensive environment. The basis for harassment includes but is not limited to age, color, disability, national origin, race, religion, sexual orientation, gender identity, or sex/gender. Sexual harassment is serious and complex. It includes any deliberate or unwanted verbal or physical sexual contact, sexually explicit derogatory statements, or sexually discriminatory remark that is offensive or objectionable to the recipient, causes the recipient discomfort or humiliation, or interferes with the recipient’s performance. Sexual harassment can take the form of any unwanted sexual attention, ranging from leering, pinching, patting, verbal comments, display of graphic or written material, and subtle or expressed pressure for sexual activity. In addition to the anxiety caused by sexual demands on the recipient, sexual harassment may include the implicit message from the alleged offender that non-compliance will lead to reprisals, such as harassment escalation, unsatisfactory evaluations, the difference in treatment, sarcasm, or unwarranted comments to or by peers. Abuse of a minor requires additional emphasis. Improper verbal, visual, or physical conduct or sexually related behavior involving minors will not be tolerated, including inappropriate social media activity related to the FLAG environment. It is the policy of FLAG that a sexual relationship between staff or adult volunteers and anyone under 18 years old is not permissible in any form or under any circumstances. Bullying is deliberate or intentional behavior using words or actions intended to cause fear, intimidation, or harm. Bullying behavior can be physical (e.g., assault, threatening physical harm), verbal (e.g., threatening language, name-calling), or indirect (e.g., spreading rumors, circulating inappropriate photos, cyber-bullying). Bullying has a significant negative effect on the victim and bystanders and will not be tolerated. Other behaviors may be inappropriate even though the above definitions do not cover them. Inappropriate behavior is that which is inconsistent with the mission of FLAG and maintaining a welcoming and safe participatory environment. Any person encountering harassment, bullying, or inappropriate behavior should deal with it immediately through proper procedures. FLAG will take appropriate action to address all violations of this policy
